Signature Dental is providing notice of an event that may have impacted the protected health information (“PHI”) of a small number of Signature Dental patients. This notice provides details about the event, measures we have taken in response, and additional steps patients can take to help protect their PHI, if they feel it is necessary to do so. On April 8, 2025, approximately six boxes of paper-based dental records were taken by an unauthorized individual. We became aware of the issue the same day and immediately initiated an investigation into the event. Through this investigation, we determined that these records related to patients who were treated by Dr. Alan Levy in 2019 or earlier. We understand that the PHI contained in these records includes patient name, contact information, date of birth, medical history, dental treatment notes, medical history and imaging notes. If such information was provided by the patient, the records may also include Social Security number, driver’s license number and health insurance and payment information.

Signature Dental is committed to protecting the privacy and security of our patients’ information. In response to the event, law enforcement was notified and is undertaking an investigation. In addition, Signature Dental is reviewing and enhancing the physical safeguards that are in place to protect paper-based records. Further, while Signature Dental is not aware of any malicious misuse of PHI in connection with the event, out of an abundance of caution, we have secured the services of Kroll to provide two years of identity monitoring services to patients who were potentially impacted by the event at no cost.

Patients who believe they may have been impacted by the event and wish to sign up for identity monitoring services, or who otherwise have questions about the event, may call 1-888-284-9929 Monday through Wednesday, from 7:00 a.m. to 4:00 p.m. Pacific, Thursday from 7:00am to 2:00 p.m. Pacific, and Friday from 7:00 a.m. to 5:00 p.m. Pacific, excluding major U.S. holidays.

As a best practice, we encourage potentially impact patients to remain vigilant to the possibility of fraud or errors by reviewing account statements. We also recommend monitoring free credit reports for any unauthorized activity and reporting any such activity.
